The consumer’s consent to electronic records and signature applies to the signing of the insurance application, waivers and any applicable endorsements with Capitol Insurance Company. The consumer may print or download the electronic record. If you have any problems signing the document, printing, downloading, or an error occurs in the transmission of the electronic record, please immediately call our customer service team at 800-394-2423.
The consumer has the right or option to have the electronic record provided in paper or non-electronic form. At any time, the consumer may withdraw the consent to have the records provided or made available in electronic form. In the event the consumer elects to withdraw his consent, the consumer can contact the company’s customer service department at 800-394-2423 and request a paper copy of the electronic record. There will be no fee associated for obtaining a paper copy of the electronic record. In the event, there was a discount associated with the consent to electronic records and signature, the discount may be reversed.
If the consumer requests a paper copy of the electronic record and notified the company by contacting the customer service department, the company will send the paper copy via United States Post Service regular mail.
The withdraw of consent by a consumer will not affect the legal effectiveness, validity, or enforceability of electronic records provided or made available to the consumer prior to the implementation of the consumer’s withdrawal of consent.
In order to access the electronic records the consumer will need the following hardware and software: access to the internet; a valid email address to receive electronic records; a computer or mobile device with the necessary software to download, view and store the electronic records; and a printer, if the consumer elects to print the electronic record.
The company will provide the consumer with advanced notice, if the hardware and software requirements to access or retain the electronic records changes. By providing your Consent to Electronic Records and Signature, you acknowledge that you have access to the necessary hardware and software to receive and store the electronic records.
